Require fully explicit scaling values at every level

All 96 empty {} scaling entries now have explicit Rx values so external
implementations can read each level independently without fallback logic.
Converted 12 gender-specific object sub fields to simple strings. Updated
spec and playbook rules to forbid empty objects and object subs. Added
audit checks to catch these in future metcon generation.
